Mining
Forms
- Used annually to certify that the required
annual labor was done on each mining claim. This document
must be timely recorded in the appropriate recording district
office. For additional information see the Annual
Labor Fact Sheet. To change a recorded affidavit
you will need to record an Amended
Form.
Affidavit
of Initial Production for Mining
- Used to meet the State of Alaska's regulatory
requirements to record an Affidavit of Initial Production
for a new mining operation.

Placer Mining Application and Related Forms (APMA's)
Production
Royalty on Locatable Minerals
- Any miner who received a mining license
in the previous year or if you are a claim owner, operator,
lessee, or royalty interest recipient; you are required to
file these forms by May 1. For additional information

Quit
Claim Deed for Mining Claims Form
- Used to transfer ownership of a mining
claim. This form must be recorded in the appropriate recording
district office.
Rental
Calculation Worksheet for Mineral Locations on State-Owned
Land
- Used to calculate the first rental payment
due at the time of recording a new location certificate/notice.
State
Mining Claim MTRSC Location Certificate/Notice
- Used as a Posting Notice on new MTRSC
mining location based on legal description. One copy goes
on the appropriate corner post of the claim and one copy
is recorded in the appropriate recording district. For more

New Locations Fact Sheet. To change a recorded certificate
you will need to record an Amended
Certificate.

Mining Claim Traditional New Location Certificate/Notice
- Used as a Posting Notice on new NON-MTRSC
mining location based on the traditional method of locating
on the ground. This is only for 40 acre claims that do not
conform to MTRSC requirements. One copy goes on the appropriate
corner post of the claim and one copy is recorded in the
appropriate recording district. For more information see MTRSC

you need will to record an Amended
Form.

Prospecting Site New Location Certificate/Notice
- Used as a Posting Notice on new prospecting
site location. One copy goes on the appropriate corner post
of the claim and one copy is recorded in the appropriate

certificate you will need to record an Amended
Form.

for Water Rights
- Used by a property owner to initiate, develop,
and perfect a water right for the beneficial use of water from
a specific source of surface or ground water. For additional

Application
for Temporary Water Use
- Required for the temporary use (under 5 years)
of a significant quantity of water as defined by law. There
are no water rights associated with this permit. For additional
information see the Water

Change
of Address for Water Rights
- Used to notify the Water Management Section
of an individuals change of Name or Mailing Address.
Change
of Property Ownership for Water Rights
- Used to notify the Water Management Section
of a change in the ownership of property where a water right
permit/certificate is established.
Hazard
Classification and Jurisdictional Review Form
- Used to determine the downstream hazard
classification of a dam in accordance with 11 AAC 93.157 and
to determine if a dam is under the jurisdiction of the Alaska
Dam Safety Program based on the definition of a dam articulated
under Alaska Statute 46.17.900(3).
Notice
of Relinquishment of Water Rights Form
- Must be signed by the owner of the property
with a water right permit/certificate when the owner is voluntarily
relinquishing all or part of the property's water right. This
form must be notarized. For additional information see the Water

Request
for Water Right Permit Extension
- A one time permit extension may be granted
for cause if the water right is not fully developed by the expiration
date on the permit. For additional information see the Water
Rights in Alaska Fact Sheet.
Statement
of Beneficial Use of Water
- Must be signed by the property owner to certify
that water use on the property is fully developed. Once the
water use if fully developed, a permanent water right certificate
may be issued. This form must be notarized. For additional information
see the Water Rights
in Alaska Fact Sheet.
Water
Well Record (Driller's Well Log) Form
- Well Drillers are required to submit to the
Division, a completed "well log" of each well drilled.
