Reporting
Access the App for ADV Reporting:

For mobile use download the Survey123 field app to your mobile device. For desktop use simply click the link in #2 and then click open in browser.

Once you have downloaded the app, link this survey to the app by clicking the click below.
Link Your Device - Start using the app to report ADVs.
Note: The Survey123 field app can be used offline.
If you don't have cell service, you can still use the mobile app to collect a survey and save as a draft. When you are back into cell service add the locational data by typing in the latitude and longitude at the bottom of the basemap or by dropping the pin on the base map where the survey was collected. Then hit the check button to submit the survey.
